چکیده

observed 12 month height velocity at the mean value for the population-'that is, 6 27 cm a year on standard charts-the 95% confidence interval for his true velocity ranges from the 25th centile to the 75th. However, if another boy aged 2 years also has an observed velocity on the 50th centile-8-31 cm a year in this case-the 95% confidence interval ranges from the 21st centile to the 79th. This degree of imprecision in determining the rate of growth, at any age, means that in practice the calculation of a single velocity, even over 12 months, is of limited clinical use.3 The measurement of height must therefore begin at the earliest possible age and be repeated at frequent -intervals to observe the trend, ideally by the same observer. The data for each child measured should be plotted on an individual height chart, and an explanation sought for unexpected changes in centile.

Severe hypernatraemia (221 mEq/l), rhabdomyolysis and acute renal failure after cerebral aneurysm surgery.

Hypernatraemia is a relatively frequent electrolyte abnormality in hospitalized patients, with reported incidences ranging from 0.65 to 2.23% [1]. Normally, when the serum sodium rises, thirst develops and antidiuretic hormone (ADH) is secreted. Hypernatraemia will not occur in individuals with an intact thirst mechanism and free access to water, due to a potent osmolar stimulus to drink. Hypot...

The dilemma of diagnosing the cause of hypernatraemia: drinking habits vs diabetes insipidus.

Fortunately, hypernatraemia is not a common problem, occurring in <1% of patients in an acute care hospital. It is serious, however, as hypernatraemia is correlated with a high mortality rate [1]. A major reason that hypernatraemia is so rare in conscious adults is the presence of powerful, highly regulated responses to a rise in plasma osmolality, namely thirst and anti-diuretic hormone (ADH) ...

Severe Hypernatraemic Dehydration and Unconsciousness in a Care-Dependent Inpatient Treated with Empagliflozin

A 66-year-old Caucasian male became unconscious 2 weeks after initiation of add-on therapy with empagliflozin for poorly controlled type 2 diabetes mellitus. The inpatient had recently suffered focal pontine stroke, rendering him bedridden and requiring increased nursing care, including assistance with drinking. The patient had received empagliflozin 10 mg once daily for glycaemic control. Inve...

Frequency and type of renal and electrolyte disorders in fulminant hepatic failure.

Of 48 patients with fulminant hepatic failure who progressed to grade III or IV encephalopathy 38 showed evidence of renal impairment. In 32 of these patients the underlying cause could be placed initially into one of three categories-prerenal uraemia (4 patients), acute tubular necrosis (16), and "functional renal failure" (12). The latter differed in several respects from that seen with liver...

A 6-month-old female Staffordshire bull terrier cross presented to the Onderstepoort Veterinary Academic Hospital with lethargy, weakness, decreased appetite, abnormal vocalisation, twitching of the facial muscles and ears, circling, alopecia, pruritus, seborrhoea sicca and erythema. Serum biochemistry revealed severe hypernatraemia (200.4 mmol/L; Reference Interval 140 mmol/L - 155 mmol/L). Tr...
